I did some digging in the MF Scan Utility folder and found 2 applications: MFSCANUTILITYL.exe (note the "L" before .exe) and also MFSCANUTILITY.exe. The first is the app I get when I type MF Scan utility into Windows search. And scanning had always failed with it. I then double-clicked on the MFSCANUTILITY.exe file instead, and got a different GUI.
